Your First Visit

Your initial appointment will consist of a comprehensive consultation where Dr. Brown explains your diagnosis and all available treatment options. In some cases, treatment can begin the same day — though complex medical histories or treatment plans may require a follow-up appointment.

Patients under 18 years of age must be accompanied by a parent or guardian for their first consultation appointment. Please also inform us prior to your visit of any medical conditions (diabetes, heart conditions, joint replacements, blood thinners, etc.) or medications that may affect your treatment.

  • Valid photo ID — a government-issued photo ID such as a driver's license or passport
  • Dental insurance card & forms — bring your insurance card and any completed patient registration forms to speed up processing and help us maximize your benefits
  • Current medication list — a complete list of all medications, supplements, and vitamins you currently take, including dosages; this is essential for your safety
  • Recent dental X-rays (if available) — if your previous dentist has taken X-rays within the last 6 months, request they be forwarded to our office or bring them with you
  • Referral slip (if applicable) — if you were referred by another provider, please bring the referral slip or documentation

What is your cancellation policy?
